New Client Surety Application and Forms

An initial application for bonding will allow us to run credit reports, and will give us a pretty good idea of how much bond credit you qualify for, as well as which surety would best meet your needs.

The application should include:

  1. Contractor Questionnaire, completed and signed.
  2. The most recent YEAR-END FINANCIAL STATEMENT, preferably prepared by a CPA according to GAAP (Generally Accepted Accounting Principles).
  3. Two prior YEAR-END FINANCIAL STATEMENTS, if you’ve been in business long enough.
  4. Copy of your most recent COMPANY TAX RETURN.
  5. Personal Financial Statement(s) on the Owner(s) and spouse(s), as of the same date as the most recent company financial statement.
  6. Year-end or recent financial statements on related companies, if any.
  7. Resumes of the owners, officers and key personnel.
  8. Bank Reference. Fill out top of form and send to your banker for completion.
  9. Copy of a current Certificate of Insurance.
  10. WORK-IN-PROCESS. If you have the same information on a spreadsheet and/or generated by your accounting software, you may provide that.
